Freud fue pesimista acerca del análisis en las neurosis narcisistas. Pensó que los que sufren estas enfermedades, no tienen o sólo insuficiente capacidad para la transferencia. Describió la resistencia de estos pacientes como un muro de piedra que no puede moverse, y dijo que se apartan del médico, no por hostilidad, sino por indiferencia. Muchos autores han tratado de desarrollar métodos de análisis para manejarse con pacientes narcisistas, Waelder (1925), Clarck (1933) y posteriormente From-Reichmann (1943, 1947), Bion (1962), Rosenfeld y otros.
