El caso Schreber; una contribución a la esquizofrenia, hipocondría y a la formación de síntomas psicosomáticos

1) La literatura sobre el caso Schreber es repasada brevemente. Las conclusiones de Freud han permanecido sin ser examinadas. No se ha estudiado ningún material nuevo de las Memorias ni se han formulado nuevas interpretaciones.
2) Se señalan con énfasis debilidades y discrepancias con el estudio de Freud, debido a la falta de discriminación entre neurosis y psicosis. Los delirios hipocondríacos (síntomas somáticos) son pasados por alto por Freud y en general por la teoría y práctica psicoanalítica.
3) A la vista de esto, la literatura sobre fantasías de embarazo es analizada críticamente. Un énfasis excesivo en los aspectos genitales, homosexual y neurótico ha llevado a pasar por alto los mecanismos psicóticos, es decir, la irrupción de fantasías de procreación arcaicas, pregenitales y asexuales.
4) Un nuevo examen del material de Schreber muestra que su psicosis es una reactivación de fantasías de procreación arcaicas asexuales, con la pérdida concomitante de diferenciación del sexo. Era tanto masculino como femenino, ambos y ninguno. El conflicto homosexual libidinal explica únicamente el aspecto genital del cambio de sexo, sin tomar en cuenta el
aspecto creativo psicótico fundamental de transformación en mujer reproductiva.
5) La absoluta ambisexualidad de Schreber representa el “desequilibrio equilibrado” del sexo que con regularidad, sino invariablemente, se encuentra en los esquizofrénicos.
6) El análisis de Freud es analizado a la luz de estos descubrimientos.
7) A pesar de que mucho del material de Schreber puede ser interpretado por la expresión esquizofrénica, la clave de su psicosis se encuentra en los delirios hipocondríacos que son y expresan la capa más profunda y quebrantadura de sus fantasías de procreación.
8) Se discute el mecanismo psicótico que sostiene a la hipocondría y su relación con la psicosis; se señala su posición nosológica y su relación con la formación de síntomas psicosomáticos.
